Underage Consent of Parent or Guardian

I understand, accept and consent to the terms and conditions of enrollment as written above. In particular, as Parent/ Guardian of the above-named student, I hereby release Roundabout Driving School and any affiliated family members, or companies owned by Roundabout Driving School, together with any employees or representatives from all liability for, emotional or physical injury, death, liability, property damage, costs and expenses.

I hereby waive any claims or rights of action of any kind arising from the above- named student's participation in the training provided by Roundabout Driving School and further agree to hold harmless Roundabout Driving School and any of its affiliated family members and /or companies together with any or all representatives of Roundabout Driving School.
